Dr Shehu I agree that nutritional optimization is particularly important in this patient. His severe dysphagia and cachexia are not simply consequences of the cancer but may also prevent him from tolerating definitive treatment. I would therefore initiate nutritional assessment and support early, while expediting endoscopic biopsy and staging.
