Where does data flow break down in your system, and why?
In your response (150–250 words), consider:
- How data is currently captured (paper, digital, or hybrid)
- At what point does the workflow break - where information is lost, delayed, or inaccurate?
- What data is missing or delayed?
- What impact does this breakdown have on patient care, reporting, or decision-making
- Suggest at least one mitigation strategy.
