Step one: Think of a diagnosis you've seen — or could imagine seeing — written vaguely in a patient chart. Something like "Infection." "Breathing problem." "Kidney disease." Write it down exactly as it might appear in a poorly documented record.


Step two: Now rewrite that same diagnosis using three elements — Cause, Severity, and Clinical Indicators. What caused the condition? How severe is it? And what clinical evidence supports it? Just like we saw "Heart failure" become "Acute decompensated congestive heart failure with reduced ejection fraction" — I want you to make that same transformation with your own example.
